Lamictal for Bipolar (Lamotrigine)

by Sarah from Bipolar-Lives
(US)

US BRAND NAME: Lamictal

GENERIC NAME: Lamotrigine

TYPE: Anticonvulsant and Mood Stabilizer

USE: Treatment of bipolar depression. Its main use originally as to control seizures in the treatment of epilepsy. It is now known to be effective in the treatment of bipolar disorder. Lamictal is important because it treats bipolar depression without triggering mania, hypomania, mixed states, or rapid-cycling. It has not demonstrated efficacy in the treatment of acute mania.

COMMENTS: An emerging gold standard in treating bipolar is a combination of Lamictal and lithium. Popular because it is very effective as an antidepressant and is almost unique amongst bipolar meds in that it does not cause weight gain.

There is a low has extremely low risk of side effects - other than the potentialy deadly rash.

According to Dr Jim Phelps:
"Unfortunately, it does not have reliable anti-manic effects, so it's not enough, alone, for Bipolar I. But for Bipolar II, it can be fantastic -- indeed, the holy grail: 100% symptom control, 0% side effects."

Lamictal is a good choice where your main symptom is depression and you are afraid of weight gain.

In terms of the rash, it is more leikely to be the severe SJS type if it appears above the neck, around or in the mouth, in soft tissue such as mucous membranes of mouth, nose, eyes, or if it appears on the face. Below the chin is more likely to be an "ordinary" rash which may go away if you are patient and reduce your dose for a while before slowly increasing to therapeutic levels.

EVIDENCE: According to Consumer Reports and the British Medical Journal, people with bipolar depression who take lamotrigine are more likely to find that their symptoms get better or go away. Evidence shows the benefits usually outweigh the harms.

SIDE EFFECTS: Include but not limited to: Skin Rash, Blurred vision, Diarrhea, Dizziness, Double vision, Headache, Nausea.

CAUTIONS: Along with the risk of an unpleasant skin rash, there is also risk of SEVERE rash including Stevens-Johnson syndrome - this can be fatal. Very low starting dose and very slow increase of dose seems to be best way to prevent any of the skin rash side effects.

Lamictal, felt good but body didnt like it
by: Anonymous

I tried Lamictal for a week, only 25 mg and i had a very slight rash but more of a problem was the sever hot flashes i was getting. i was feeling pretty good too but was so scared of getting the deadly rash that i stoppped taking it. i was red and splotchy all over but had no temperature. im not sure if it was caused by the lamictal but doc made me stop. 8 months of gabapentin not helping i feel like im never going to have a normal life again. im wondering if taking half that of lamictal and starting slower would help or if im just allergic. i just want to peace in my life and im so tired of the severe depression i go through and the constant aggitation with racing thoughts. i also do not want to gain more weight, i gained 20 lbs last year and havent been able to get it off YET!

Effects of Lamictal
by: S. Sigler

Lamictal has been nothing short of wonderful for me...as long as it's combined with an antidepressant/s. As with everything else, I seem to have the opposite reaction to lamictal as do other people. For me, it works for controlling my mania completely, without dampening my mood. My doctor tried me on it alone, and it really didn't help my depression but a little. It didn't work at all with Effexor. NOTHING works with Effexor, lol. I still had very serious hypomanic symptoms, but I don't believe it was lamictal's fault. Now I'm on a cocktail of Wellbutrin, Celexa, and 200 mg. Lamictal, and it's the best I can remember ever feeling since being diagnosed. No negative effects whatsoever. When I first started, it had just been approved by the FDA, so I was on the brand name product, and I needed a prior authorization to get it. When they were able to come out with generic versions, of course my insurance would only accept the generic (lamotrigine). This is a warning about the generic versions. They can very tremendously in effectiveness and side effects. That is because, apparently, generic brands can differ in their formula as much as
30%! My insurance company changed brands 3 times on me. First they paid for Teva Labs, which was excellent. Then to Apotex which equally as good. But then they switched to Mylan, and it was horrible! It made me dizzy, nauseated and feeling crazy, like a mixed episode. I tried a different pharmacy and they had Dr. Reddy's Lab...?, and it was just as bad. Thankfully, my insurance finally agreed to let me have 25 mg. chewables from Teva brand. I have to take 8 a day, lol. But they're tiny, don't taste that bad, and they WORK!

Lamictal - the Real Wonder Drug
by: EAC

200 mg of Lamictal per day has been a wonder drug for me. I have been prescribed countless psychotropic drugs over the last 11 years, and this has been the best. However, I still have trouble focusing and controlling irritability, and wonder if low dose Lithium would help me. Many years ago I remember trying some and though I have trouble remembering much during that time due to my illness, I believe I was taken off of Lithium for severe diareah. I wonder if anyone else has had that trouble and if a lower dose helped.

Lamictal Rash
by: Anonymous

I tried it on low dose for 4 days, but got one big dirty looking crater of a hive/pimple like thing on my chin. Was in a manic phase at the time, so had to come off it and go back to Zypreza. Was this the BAD rash? or the one that might have gone away? Would have liked to stay on it otherwise.

Sounds great (except for the rash)
by: Sarah from Bipolar-Lives

Have heard many good reports of Lamictal, especially when used with lithium or another med that has good anti-manic properties. I got the rash and gave up on Lamictal but I would love to be able to take it. (Most people have no side effect problems so don't be afraid to give Lamictal a try.)
